Requirements To Participate

- Adhere to the Good Conduct Policy within the Ar-We-Va Student Handbook

- Iowa Athletic Pre-Participation Physical Examination Form completed by a physician or chiropractor to confirm eligibility of student athlete +  questions answered and signed by student and parent

- Heads Up Concussion Clearance Form confirming key information about topics related to this injury completed by parent and student

​

- Insurance Verification Form confirming medical and accident insurance is in place for student athlete and typically completed by parent
